Membership Programs

How Direct Primary Care Membership Works

Covered Services For Members:

 

The practice’s services can be divided into two categories based on (1) Preventative care/medical management and (2) unplanned/sick visits. Memberships are available for those 18 years of age and older.

Preventative care and medical management include almost anything seen, evaluated and treated in a conventional Primary Care office, including screening exams (Annual physical), Pre-operative evaluations, medication management, chronic disease management, periodic consultations for management of chronic conditions ranging from hypertension, diabetes, hyperlipidemia, endocrine disorders (diabetes, thyroid, low testosterone), peripheral vascular disease, arthritis/gout, mood disorders/anxiety, insomnia, allergies, asthma, COPD, headaches/migraines, weight issues, fatigue, among other typical primary care complaints.

 

Researching symptoms and concerns, arranging outside testing when appropriate, contacting and communicating with specialists on behalf of the Patient, as well as reviewing and explaining medical records.

Unplanned/sick visits include a long list and wide range of conditions too extensive to fully list, but commonly include URI Sx, suspected viral syndromes and seasonal illnesses, many acute worrisome symptoms such as cough, fever, weakness, dizziness, abdominal pain or changes in bowel habits, UTI Sx, injuries, rashes/envenomation.

Preventative care and medical management

Includes almost anything seen, evaluated and treated in a conventional Primary Care office, including:

​

  • Screening exams (Annual physical)

  • Pre-operative evaluations

  • Medication management

  • Chronic disease management

  • Hypertension

  • Diabetes

  • Hyperlipidemia

  • Endocrine disorders (diabetes, thyroid, low testosterone)

  • Peripheral vascular disease

  • Arthritis/gout

  • Mood disorders/anxiety

  • Insomnia

  • Etc etc

 

Allergies asthma, COPD, headaches/migraines, weight issues, fatigue, among other typical primary care complaints. Researching symptoms and concerns, arranging outside testing when appropriate, contacting and communicating with specialists on behalf of the Patient, as well as reviewing and explaining medical records.

Unplanned/Sick Visits

 

Includes a long list and wide range of conditions too extensive to fully list, but commonly include:

​

  • URI Sx

  • Suspected viral syndromes

  • Seasonal illnesses

  • Cough

  • Fever

  • Weakness

  • Dizziness

  • Abdominal pain

  • Changes in bowel habits

  • UTI Sx

  • Injuries

  • Rashes/envenomation

In office testing – EKG, UA, Pregnancy test, Rapid strep, Glucose via fingerstick, Covid/Flu, and more...

​

Covered Treatments:

 

Always depends on condition and diagnosis. Minor procedures, including simple laceration repair, trigger point injection, available lab testing; administration of vaccines and Vitamin B12.

​

Non-Covered Treatments:   Anything not specifically listed as a “Covered Services”, including house calls, complex laceration repair, durable medical equipment (Braces, Splints, Crutches, etc)

Membership Rates:

​

Enrollment fee – (one time) $60.

​

Membership fee – (monthly): $60 individual/$100 couple.

​

Non-Membership rates:

​

Initial visit/consultation $180

​

Follow up office visit $120

 

Same day (or next day) office appointments are available for members, as well as after hours availability via text, phone calls, and telemedicine for more urgent concerns. Not all conditions can be handled remotely so a text, portal message, phone conversation etc, may lead to an office visit, which are typically 30-60 min. Such decisions, when appropriate, will be made at the sole discretion of the physician.

Membership ensures patient access by text, phone call, telemedicine, and office visit for ongoing medical care (the number of office visits per month will depend on individual needs), including consultations (described below), annual visits for Physical exams.  The agreement can be terminated via 30 day notice at any time, by either patient or physician, for any reason, and immediately with breach of contract by either party. An appropriate refund will be made at that time as well.

 

The DPC agreement is a private contract for covered services between the patient ( or legal representative) and the physician, and it is designed to maximize the doctor-patient relationship by eliminating the insurance and government involvement and interference. BUT the agreement is not itself a form of “insurance”, and the periodic fee is not intended in any way to anticipate an illness or injury; in fact, the best arrangement is for a DPC member to also have a HDHP to cover costly injuries, needed referrals, hospitalizations etc.
